[Clinicians] Travel Reimbursement for Clinicians to Attend Annual Conference

Hello CHAMPS Clinicians'' Listserv Members,

 

This is a reminder that Region VIII clinicians, who are members of
CHAMPS, are each eligible for $700 in travel reimbursement for the
CHAMPS/NWRPCA Annual Primary Care Conference.

 

Attached is the new CHAMPS Clinical Travel Support Reimbursement Request
form.  This form reflects the new per diem rates that will be in effect
as of October 1, 2009.  Please use this form when submitting your
request, and be sure to read the bottom of the form to assure you meet
the eligibility requirements, such as working .5 FTE or greater.

 

If you have not yet registered for the conference, please see the link
at the bottom of this email for information about the conference
sessions, accommodations, and registration.
