Hello CHAMPS Clinicians' Listserv Members,

 

Please see the information below regarding the HRSA Patient Safety &
Clinical Pharmacy Services Collaborative.  HRSA is placing a strong
emphasis on this collaborative and is encouraging CHCs who have not
already done so to consider participating.  Also, the Office of
Performance Review (OPR) in Region VIII will have a pharmacist, Dr.
Christina Mead, joining their staff in June.  She should be an excellent
resource for addressing pharmacy related issues.

This is an opportunity that you don't want to miss!!

 

You are being invited to join a national breakthrough effort that has
been underway for over a year, transforming how communities deliver care
to patient populations with poor health status and high medication risk.
A year of action learning through the Patient Safety and Clinical
Pharmacy Services Collaborative (PSPC) has produced a knowledge and
implementation platform that your community can adapt and use today. You
can use it to dramatically improve the health status and safety of your
patients with multiple chronic diseases, high risk drug therapy, complex
drug regimens and multiple care delivery sites.  In this collaborative
you can rapidly take major practical steps to provide patients
integrated care in a primary healthcare home with sustainable and
measureable clinical pharmacy services. 

 

The Health Resources and Services Administration (HRSA) invites
community based teams of health care providers to join Version 2.0 of
the Patient Safety and Clinical Pharmacy Services Collaborative (PSPC
2.0).  The mission of this Collaborative, which is modeled after the
Institute for Healthcare Improvement's Learning Collaboratives, is to
advance the delivery of world class care by spreading the integration of
clinical pharmacy services and patient safety principles to improve
health outcomes in safety-net populations.

 

HRSA invites you to form a team with other health care organizations in
your community to take part in this exciting and unprecedented
opportunity!

 

The PSPC Participation Package is NOW available at 
www.hrsa.gov/patientsafety.  More information about PSPC and FAQs on the
Participation Package are also available at this website.  The
Participation Package can also be found on the HRSA Knowledge Gateway at
