Hello CHAMPS Clinician's Listserv Members,

For those of you who may have missed the email sent by Direct Relief
International last week, TEVA Pharmaceuticals recently made a donation
of ProAir(r) HFA Inhalers to assist patients in safety net practices.

Direct Relief International, the National Association of Community
Health Centers (NACHC), and the National Association of Free Clinics are
working together to help distribute this donation. To be eligible to
receive donated inhalers, CHCs must complete a questionnaire available
at:

http://www.directrelief.org/PartnerPrograms/DomesticPrograms/ProAirDonat
ion.aspx?terms=inhalers

Log-in: TevaOffer
Password: dritevaoffer

CHCs that responded to the e-mail sent by Direct Relief International
last week do not need to fill out the questionnaire again. Requests must
be received by end of day Wed., Feb. 4, 2009.
