Center Operations
This session will provide an overview of the principles and practices of
Quality Improvement, Risk Control, and Patient Safety, and how these
principles apply most realistically to the health center setting. Tools
and methods of how to identify important risk trends, contributing
factors, and best solutions will be discussed.

Lifestyle Changes: Helping Patients Manage Diabetes
Type 2 Diabetes Mellitus has a strong genetic background but develops in
susceptible people mostly as a result of poor dietary habits, physical
inactivity and overweight. The Director of Endocrinology and Diabetes
Practice, at the University of Colorado Health Science Center, will
discuss recommendations for lifestyle changes along with the use of
medications for preventing and treating Diabetes.
Primary Care Renewal: Creating a Primary Care Medical Home Through a
Learning Collaborative Sponsored by a Health Plan
The first 18 months of a journey in creating a primary care medical home
in five diverse organizations, based on the work done by Southcentral
Foundation, will be presented. This work, sponsored by CareOregon, a
Medicaid Managed Care Organization, uses the model of a learning
collaborative.

Behavioral Health in Primary Health Care Settings
With ongoing cuts in funding for mental health and continuing
stigmatization of mental disorders in our country, it is more important
than ever to connect people to the services they need. Learn more about
methods and reasons to integrate behavioral health into your health care
settings from a Clinical Social Worker who works "in the trenches" of a
CHC behavioral health department.
Substance Abuse in the Primary Care Setting: A New Perspective
This presentation will focus on the treatment of substance abuse as a
chronic medical illness, using lessons learned from the chronic care
model, from a community care clinic medical director. A team approach
to treatment will be emphasized along with the "harm reduction"
philosophy. Best practices for the medical management of substance
abuse will be discussed.

Rural Healthcare Ethics: Some Practical Issues Utilizing Cases
This interactive session will feature two members of the Coalition for
Rural Healthcare Ethics leading a discussion of topics and cases in
clinical ethics in rural settings. It will offer a preview of the
resource Handbook for Rural Healthcare Ethics currently being developed
by the Coalition. Participants will be encouraged to bring their
personal experiences with ethical challenges to the discussion.
Managing Chronic Pain Patients When the Magic Cure Has Ended and They
Are Not Getting Better
CHC providers often feel inadequate and/or overly burdened when managing
the multitude of chronic complaints that rarely improve. Chronic pain
patients often want a MAGIC cure and when there is none they feel
hopeless, despondent, and lose function. This session will assist with
how to address such difficult issues from a CHC clinician, with a strong
interest in chronic illness and pain, who currently manages hundreds of
chronic pain patients.

Providing Culturally Competent Care for the Gay, Lesbian, Bisexual,
Transgender Population (GLBT)
This session uses the power of theatre and an in-depth physician lecture
to explore health disparities of the GLBT population. On The Streets is
a play created and performed by the Kaiser Permanente Educational
Theatre Program to express the care needs of the GLBT population. The
performance is followed by a GLBT culturally competent care training
with a pediatric physician and a general internal medicine physician.
