[Clinicians] Travel Reimbursements for the Oct 2008 Primary Care Conference Due Dec 15th

Hello CHAMPS Clinician's Listserv Members,

 

I wanted to remind you that CHAMPS does award travel reimbursements to
Region VIII clinicians who traveled to the CHAMPS/NWRPCA Fall Primary
Care Conference of up to $700.  I have attached the Reimbursement
Request Form.  There is a paragraph at the bottom of the form about
eligibility for this reimbursement.  

 

If you would like to request reimbursement, please fax the attached form
to CHAMPS at 303-861-5315 no later than Monday, December 15, 2008.
